Primary Care Services
DDHA operates medical clinics for adults with intellectual and developmental disabilities (IDD) using care teams of nurse practitioners and physicians. A unique health home model is utilized which integrates primary care, mental health and seizure management services. This health home model serves individuals who are registered with the New Jersey State Division of Developmental Disabilities (DDD) and receive Medicaid and/or Medicare benefits. These clinic services are currently available in two locations (Bloomfield and Toms River) and are supported by clinicians who provide on-call services on a 24/7 basis. Access to primary care services can be arranged by contacting one of DDHA’s offices. Each office is staffed by a nurse practitioner, physician and additional dedicated support staff .
