Primary Care Services

DDHA operates a medical care center for adults with intellectual and developmental disabilities (IDD) using a care team of a nurse practitioner and physicians. A unique health home model is utilized which integrates primary care, mental health and seizure management services. This health home model serves individuals who are registered with the New Jersey State Division of Developmental Disabilities (DDD) and receive Medicaid and/or Medicare benefits. These services are currently available in Toms River and supported by clinicians who are on-call on a 24/7 basis.
